Purpose of Webinar

To promote quality end-of-life care, better communication between clinicians, patients and families is a priority.

Educational Objectives

Talking about death is hard for almost anyone and clinicians and caregivers are no exception. This program will provide guidance on how to initiate these difficult conversations in the right way, at the time. Melissa Abbott of 5 Star Consultants is the presenter for this session. Specific learning objectives include: 

  • Enhance understanding of the importance of advanced care planning and having the conversation.
  • Learn ways to initiate goals of care conversations and the importance of timing.
  • Enhance communication with patients and families.

About the Educator – Melissa Abbott, RN, MSN, MHA, 5 Star Consultants 

Melissa is a registered nurse with experience in a variety of health care settings with the majority of her nursing experience in home health care and hospice where she was a clinician working in the field and then later became an administrator of a hospital-based home health and hospice agency. In 2017, she joined 5 Star Consultants as a home care and lead hospice consultant. 

Melissa has an expertise in identifying issues, solving problems and implementing action plans that are successful. She also has a passion for individual coaching, team building and leadership development. Abbott has served on the Iowa Alliance in Home Care Board of Directors and the Hospice and Palliative Care Association of Iowa Board of Directors since 2014 and has been a part of advocacy work for home health and hospice within her home state of Iowa as well as nationally.
